Agency Contracting

Med James, Inc. requires following prior to contracting.

1. Completed Med James Agency Questionnaire and Producer Agreement
2. Completed W-9 Form
3. Copy of Errors and Omissions declaration page with minimum occurrence of no less than $1,000,000
4. Copy of Agency Licenses
5. Copy of Producer Licenses
6. Email completed(signed) copies of the required documents as attachments to compliance@medjames.com.

NOTICE: By submitting to us the requested documents by fax or electronically, you agree that they and copies thereof may be used by us as if they were the originals thereof in any professional, administrative or legal proceeding.
